Vena caval opening
oesophageal opening
aortic opening (kinda, not actually through the muscle itself)

What creates the upper oesophageal sphincter in the diaphragm?
A
The right crus folds over and around the oesophagus creating a loop of muscle like a sphincter.

What spinal nerves make up the phrenic nerve?
A
C3,4,5 - mostly C4
C3,4+5 keep the diaphragm alive.
