Environmental factors in genetically susceptible individuals
What is the immune response in psoriasis
Keratinocytes are under stress - release factors to stimulate plasmacytoid dendrites cells - produced IFNA
Chemical signals activate DC - migrate to lymph nodes to present and activate T cells - TH1 AND T17
T cells are attracted to the dermis by chemokines and secrete iL-17A, 17f/22 - this secretes KC proliferation, AMP release and neutrophils attacking chemokines
Signs
Symmetrical scaly plaques at extensors
Scalp and nails
Well demarcated, scaly, erythematous plaques
Nails: oncyholysis, pitting, dystrophy, subungal keratinotosis
What is koebner phenomenon
Psoriatic lesions on previously unaffected skin
What is aispitz sign
You scratch and you will bleed a little :(

1st line: topical therapies
2nd: phototherapy
3. Systemic therapy

EMOLLIENT Vit d analogies Calcipotriol ( for localised plaques) Calcitriol Coal tar Dithranol
1st line will depend on the patient
