Open lesion between the knee and the ankle that remains unrelated for 4 weeks

Medial and lateral malleoli

Pale, skinny, open, well defined, punched out

Lipodermatosclerosis
Hyperpigmentation
Woody tethered skin
What is pyoderma gangrenosum assoc with
Inflammatory response to trauma
What is necrobiosis lipoidica associated with
Diabetes
Ix of leg ulcers
ABPI ( Arterial)
Would stab ( only if it’s painful / exudate / malodour/ enlarging
Bloods ( FBC, LFT, CRP, u& e
Patch testing
Treatment
Control pain
Non adherent dressing
De - sloughing agent if necessary
4 layer compression bandaging
What are examples of de- sloughing agents
Hydrogel and honey
