What is the pathway by which maternal estrogen levels are influenced by the fetus?
Placenta converts cholesterol –> pregnenolone in placenta, Pregnenolone then goes to the fetus, where it is converted to 16-a-OH-DHA SO4 in the fetal liver, 16-a-OH-DHA SO4 is then carried back to the placenta and converted to estriol. Estriol then enters mom’s circulation
What are the hematologic effects of estrogen?
Increase clotting factors (big one)
Increase steroid hormone carrier proteins
What are the effects progesterone has on estrogen and mom’s respiratory system?
“Quiets” estrogen
Sensitizes respiratory system to CO2
What is the general effect of oxytocin?
Sm. muscle contraction: Lactation and uterine
What two systems are the primary sites of contraception side effects?
CV: increase risk of CVD
Hematologic: Increase clotting (esp with smoking)
What is Lybrel?
Contraceptive take for one year, with no breaks - no menses for one year
What are seasonal and seasonique?
Extended cycle-contraceptives
How do progesterone only contraceptives work?
Inhibit midcycle LH / FSH surge
Are estrogen only or progesterone only contraceptives more reliable?
Estrogen only
How is preeclampsia diagnosed?
HTN w/ proteinuria
How is eclampsia defined?
Preeclampsia with convulsions or seizures
What two drug classes are the hallmark treatment of preeclampsia?
Anti hypertensives, Corticosteroids
What is the anatomy behind Twin-Twin transfusion and the result?
Shared placenta leads to one baby getting the venous blood of the other –> one baby has failure or significant impairment of development
