A
- Oedema in congestive heart failure – shown to improve survival
- Ascites
- Primary hyperaldosteronism
- Nephrotic syndrome

Mechanism
A
- Inhibits intracellular aldosterone receptor -> Inhibit Na/Cl reabsorption & K+ secretion

Common: Hyperkalaemia (less commonly hyponatraemia and hyperuricaemia)
Important: Gynaecomastia/hypogonadism, impotence in males, menstrual irregularities

- Renal impairment
- Avoid in hyperkalaemia, hyponatraemia, anuria and Addison’s disease
