Management
Treatment -
Pharmacologica
IV Opioids Gold
standard
(Pain Protocol - Morphine,
Fentanyl, Ketamine)
PCA
NSAIDS
Local Anaesthetic
Important to continually
monitor for iv opioids,
airway and respiratory
function when administering IV
Opioids
Pain Management
Treatment - Communication
- Communicate openly and clearly with the patient
- Recognise and addresses common fears
- Explain procedures in a clear and understandable
manner - Provide a safe environment for patient during the all
phases of post anaesthetic care - Reassure the patient in order to relieve patient
anxiety
Other General Complications
Hypothermia - Due to exposure
- Hyperthermia - Febrile, Blood transfusion reaction,
Thyroid crisis Malignant Hypothermia
- High Spinal/Epidural Block - can lead to decrease
cardiac output
- Urinary Retention - i.e. from epidural opioids
- Hypotension - anaesthetic agents, blood/fluid loss
- Can lead to shock
- Hypertension - may be pre-existing, pain
Emergence
Delirium
‘dissociated state of
consciousness in which the
child is irritable,
uncompromising,
uncooperative, incoherent,
and inconsolably crying,
moaning, kicking, or
thrashing’.
Discharge Criteria
Must make contemporaneous record of PACU
admission and discharge
- Monitor of progress
- Record of care
- Detect complications
Discharge criteria table used to assess
readiness for
ward (score)
- Use as a handover tool
Discharge criteria table May not include
fluid balance, pain management,
temperature, wound properties, neurovascular
observations
