Advanced Care planning,
is a process in which the patient is
encouraged to discuss and identify his/her values, beliefs and life
goals, especially the healthcare trajectory he/she wishes to follow. This
can be done with those suffering from chronic and debilitating
conditions, but much more difficult with sudden and unexpected
trauma/accidents. Do you want to be intubated if you needed it?
