What is uterine prolapse?
Uterus descends into the vagina
What is vault prolapse?
Occurs in those without a uterus, when vault descends into the vagina
What is rectocele?
Defect in posterior wall - allows rectum to prolapse into vagina
What is cystocele?
Defect in anterior wall - allows bladder to prolapse into vagina
What are risk factors for POP?
Multiple vaginal deliveries Instrumental/traumatic deliveries Increasing age Obesity Chronic constipation
What are features of POP?
‘Feeling something coming down’
Palpable lump
Bowel symptoms
Bladder symptoms
What examinations are done for POP?
Pelvic exam
Speculum
Sims speculum = single speculum used to support other wall
What are the grades of POP?
0 = normal 1 = lowest part >1cm above introitus 2 = lowest part within 1cm of introitus 3 = lowest part >1cm below introitus 4 = full descent
How is POP managed?
Conservatively
- weight loss
- physiotherapy
- lifestyle changes
