Benign tumours of the soft tissue of the uterus
What are the types of fibroid?
Intramural
Submucosal
Subserosal
Pedunculated
What are intramural fibroids?
Found within myometrium
What are submucosal fibroids?
Found just below the myometrium
What are subserosal fibroids?
Found just below outer layer of the uterus
What are pedunculated fibroids?
On a stalk
What are risk factors for uterine fibroids?
Obesity
Early menarche
Increasing age
FHx
What are feature of fibroids?
Often asymptomatic
HMB
Abdo pain
Pressure symptoms
Subfertility
What is found on examination with fibroids?
Enlarged uterus
Palpable mass
How are suspected fibroids investigated?
Pelvic USS
Hysteroscopy
How are fibroids <3cm managed?
Same as HMB
Endometrial ablation
How are fibroids >3cm managed?
Symptom relief
Uterine artery embolization
Myomectomy
Hysterectomy
What is uterine artery embolisation?
Blockage of uterine artery
Starves the fibroid of oxygen so it shrinks
What is a myomectomy?
Surgical removal of the fibroid
- either laparoscopically or open
Only treatment known to improve fertility
What is fibroid degeneration?
Excessive growth of a fibroid that outmatches blood supply

Compression of fibroid feeder artery
