What is ovarian torsion?
Where the ovary twists in relation to the surrounding connective tissue, fallopian tube and blood supply
What often causes ovarian torsion?
Ovarian mass >5cm
- cyst
- tumour
What is the effect of ovarian torsion?
Ischaemia and necrosis of the ovary
How does ovarian torsion present?
Sudden onset unilateral pelvic pain
+/- N+V
+/- palpable pelvic mass
How is ovarian torsion investigated?
Pelvic USS (either transvaginal or transabdominal)
Definitive diagnosis = laparoscopic surgery
What does pelvic USS show in ovarian torsion?
Whirlpool sign
Free fluid in pelvis
Oedema of ovary
How is ovarian torsion managed?
Laparoscopic surgery
- detorsion
- oophorectomy
What is haematocolpos?
Accumulation of blood within the vagina typically due to an imperforate hymen
What are the features of haematocolpos?
Cyclical abdo pain without bleeding
What is seen on examination in haematocolpos?
Bluish tinge of membrane at introitus
How is haematocolpos managed?
Incision and drainage of hymen
What is haematometra?
Collection of blood within the uterus
What is pyometra?
Collection of pus in the uterus
