Glucagon class, indications, contraindications
Class: Pancreatic hormone, antihypoglycemic
Indications: Hypoglycaemia, hypoglycemia w/ unsuccessful IV
Contraindications: pheochromocytoma, allergy or hypersensitivity.
Glucagon dose and route
Dose (adults): 1.0mg IM
Pediatrics: 0.5mg (weight <25kg) 1.0mg (weight >=25kg)
Must be reconstituted with glycerine
ASA class, indications, contraindications
Class: platelet aggregation inhibitor
Indication: Chest pain of suspected ischemic origin
Contraindications: Severe bleeds, Stroke, head injury, asthma exacerbated by ASA, NSAID allergy, Pediatrics with viral symptoms.
ASA dose and route
Dose: 160mg PO chewed
Pediatrics: 160mg PO chewed only if patient has a history of ischemic cardiac nature.
Nitroglycerin class, indications, contraindications
Class: anti-anginal, nitrate
Indications: chest discomfort of suspect cardiac ischemia
Contraindications: SBP <90(with Rx) SBP <100(without Rx), ED drugs (viagra, Levitra in last 24hr, Cialis in last 48hr), allergy, severe anemia, pericardial tamponade, (HR 50-150)
Nitroglycerin dose and route
Dose: 0.4mg SL sprayed. Repeat Q3-5 min
Clinicall mandatory if patient doesn’t have prescription.
Naloxone class, indication, contraindications
Class: narcotic (opioid) antagonist
Indications: respiratory depression, altered mental status with suspected OD
Contraindication: allergy, neonates
Naloxone dose and route
Dose: 0.4mg, 0.4mg, 0.8mg, 2.0mg IM Q3-5
Pediatrics: 0.1mg/kg, max dose 2.0mg
D10W class, indications, contraindications
Class: carbohydrate substrate
Indications: hypoglycaemia
Contraindications: none
D10W dose and route
Dose: 10-25g (100-250ml of 10% solution) IV
Epinephrine class, indications, contraindications
Class: sympathomimetic
Indications: croup, anaphylaxis, severe bronchospasm
Contraindications: hemodynamically unstable tachyarrhythmia
Epinephrine dose and route
Dose: (Croup) 5.0mg nebulized 1-12 year old. <1 year old 0.5mg/kg to max 5.0mg
(Anaphylaxis) 0.5mg 1:1000 IM Q5 max 3 dose
(Severe bronchospasm) 0.5mg IM Q5-20
Pediatrics: 0.01mg/kg max 0.5mg IM max 3 dose
Clinicall mandatory for croup and bronchospasm
Dimenhydrinate class, indication, contraindications
Class: antiemetic, antihistamine, anticholinergic, antivertigo
Indication: nausea, vomiting
Contraindications: hypersensitive to caffeine, gravol, or Benadryl.
Patients 11 years old and younger (PCP)
Dimenhydrinate dose and route
Dose: 25-50mg IM/IV. 12.5mg for frail elderly patients.
Transexamic acid class, indications, contraindications
Class: antifibrinolytic
Indications: hypoperfusion suggestive of bleeding
Contraindications: hypersensitivity, dialysis, greater than 3 hours after the injury
