Advise - provide info and adapted advice based on the need of the person and their motivation to change
Agree - communicate in a spirit of partnership using patient-centred techniques
Assist - use various techniques to increase the patients motivation and self efficacy and address their environment
Arrange - plan the follow up check ins, provide ongoing support and refer to other disciplines
