When shall the captain make a report?
8-19-1 - Captain Report
The captain shall submit a report to the Senior Vice President, Flight Operations Center, with regards to necessary items which occurred during flight duty asap after termination of flight.
Details are specified in S-8-3
When shall a flight crew member use the ‘Voluntary Reporting System’?
8-19-2 - Voluntary Reporting System
A flight crew member should positively report events that he/she has experienced or observed, which may have caused a safety hazard, and experience that may be useful for further elimination of unsafe elements through widespread awareness to the voluntary reporting system operated by the company

When shall the company report the alcohol consumption by ‘Cabin attendants’?
8-19-3 - Report on situations involving alcohol consumption by cabin attendants
In accordance with the Civill Aeronautics Law, the company shall report situations involving alcohol consumption by cabin attendants to JCAB as situations which may affect safe operations.

S-8-3: Classification of reports
Captain and Air Safety Report definitions?
Captain report means the report which shall be done by the captain in accordance with the Law and the ones equivalent to the items designated by the law
Air Safety Report means the reports which are obliged to submit by the Civil Aeronautics Law or circular of the authorities and the ones designated by the company to prevent the future accident or increase operational quality
S-8-3 (2) - Items to be reported as Captain Report
The pilot in command shall report any of the following accidents to the Minister
i, “crash collision, or fire of the aircraft”
ii, “injury or death” of any person, or “destruction” of any object caused by the aircraft
iii, “Death or disappearance” of any person on board the aircraft
iv, made “contact with another aircraft”
v, “Other* accidents”
*Where an aircraft during flight was damaged, but excludes the cases where the damage does not need a major repair work. The damages above don’t include those limited to engines, engine cowlings or accessories, propellers, wing tips, antennas, tires, brakes or fairings.
Article 76 states
- Accidents
- Accidents occurred to other aircraft (without being reported)
- Malfunction of ground facilities or any other ground incident (Turbulence of air and other abnormal weather conditions; Vocanic eruption & other terrestrial and watery phenomena; cases obstructing the safe operation of aircraft other than those above)
- Article 76-2, Any dangerous situation that occurred on the aircraft.
How can a Captain or Air Safety Report be made?
Through the CLOVER reporting system, using facts and without delay. If it’s impossible to do so, then via a FAX after the flight, then via CLOVER upon return to base.
Immediate report shall be made by phone or orally besides reporting through the required form - to the ICP

S-8-9: Report of Alcohol for Cabin Attendants
In accordance with C.A.L. the company shall report situations involving alcohol by cabin attendants to JCAB
- Flight duty performed by CC who consumed alcohol (meaning no alcohol is in their system)
- CC who don’t properly complete the alcohol check prescribed in the OM
- Improper alcohol consumption by CC under the drinking inhibition period stipulated in the OM
