What is Obsessive compulsive disorder?
Anxiety which is associated with obsessive thoughts and compulsive ritualistic behaviour
What are obsessions?
Unwanted, thoughts, images or urges that enter the persons mind
Patient attempts to resist the thoughts
What are compulsions?
Repetitive behaviours or mental acts that the person feels driven to perform.
Patient may believe that these will prevent a bad outcome
Temporarily relieve the anxiety
Which condition is most associated with OCD?
Depression
What is the diagnostic criteria for OCD?
Person has obsessions, compulsions or both
Time consuming – causing distress in daily life
Not psychological effects of substance or medical conditions
Not better explained by another disorder e.g. dysmorphic disorder
What is the management of OCD that causes mild functional impairment?
CBT including exposure and response prevention (ERP)
What would you do if CBT wasn’t sufficient for the management of OCD that causes mild functional impairment?
Offer SSRI
or
More intensive CBT
What is the management of OCD that causes severe functional impairment?
Combined treatment with an SSRI and CBT (including ERP)
