What is cyclothymia?
Hypomania and hypo depression
What is dysthymia?
Low mood but not depression
What are the symptoms of depression?
- Changes in appetite or weight, sleep, or psychomotor activity
- Decreased energy
- Feelings of worthlessness or guilt
- Trouble thinking, concentrating, or making decisions
- Thoughts of death or suicidal plans or attempts
What are the symptoms of hypomania?
- Lesser degree of mania- no psychosis
- Mild elevation of mood for several days
- Increased energy levels and activity levels
- Increased sociability, talkativeness, overfamiliarity, increased sexual energy
- Decreased need for sleep
- May be irritable
- Concentration reduced, new interests
What are the symptoms of mania?
Elevated mood, increased energy, overactivity • Disinhibition • Grandiosity • Alteration of senses • Hypersexuality • Extravagant spending • Risk taker • Pressure of speech- speech cant keep up with their thoughts • Decreases need for sleep
How long does the episode of mania need to last?
At least 1 week
How long does the episode of hypomania need to last?
At least 4 days
What is the ICD-10 classification for the diagnosis of Bipolar Affective Disorder?
Repeated (2+) episodes of depression and mania or hypomania
How long does the episode of depression need to last?
At least 2 weeks
What is the main risk factor for bipolar affective disorder?
Strong family history- increases risk x10
What is the immediate management for hypomania?
Routine referral to CMHT (community mental health team)
What is the immediate management for mania?
Urgent referral to CMHT (also if severe depressive)
What mood stabiliser would you give for bipolar affective disorder?
Lithium
What is an alternative mood stabiliser for bipolar affective disorder?
Sodium valporate
What medication would you stop if someone was manic?
Anti-depressant
What medication would you give immediately if a person was manic?
Anti-psychotics
What antipsychotics would you give someone who was manic?
Olanzapine
Haloperidol
