MSK Spine Interventions Flashcards
Spondylosis
Degeneration of IVD
> 50 y/o
Agg: ext, standing
Ease: sitting, bending
SLR: negative
x-ray positive for arthritic changes
All āSā conditions love sitting
Spondylolysis
Break/defect in pars interarticularis in arch of vertebrae
15-20 y/o
Agg: ext, standing, bending to lift
Eases: sitting
SLR negative
x-ray oblique view shows fx
Spondylolisthesis
Slippage/forward displacement of one vertebrae over another
20 y/o; can become degenerative with age
Agg: ext, standing, bending to lift objects
Eases: sitting
SLR negative
x-ray lateral view shows slippage
Disc herniation
30-50 y/o
back, unilateral leg
agg: sitting, bending, ascending stairs
eases: ext, standing, descending stairs
SLR positive
MRI, CT indicates bulging disc
Spinal stenosis
> 60 y/o
back, bilateral leg
agg: extension, standing
eases: sitting, bending
SLR positive
CT, MRI, x-ray
Scoliosis: concave side
SP process deviates to concave side
Rib pushed ant/lat
Wide thoracic cage
Short/tight mm.
Dec lung volume
Arm in abd
Pelvic hike
glutes stretched/weak
hip ADD tight
long limb w/ supination
Scoliosis: convex side
Vertebral body deviates to convex side
Higher shoulder
Protruding scapula
Ribs pushed post (rib hump)
Narrow thoracic cage
Long/stretched mm.
Arm in ADD
pelvic drop
glutes tight
hip ADD weak
short limb w/ pronation
shoe lift
Cervical vs lumbar rotation
Cervical/thoracic SB and rotation in same direction
Lumbar SB and rotation in opposite directions
Opening restriction
Move top, stabilize bottom vertebrae
Closing restriction
Move bottom, stabilize top vertebrae
Cervical/thoracic SB opening
flexion, SB and rot away
Cervical/thoracic SB closing
extension, SB and rot towards
Lumbar SB opening
flexion, SB away, rotate towards
Lumbar SB closing
extension, SB towards, rotate away
