Flexor Synergy
No motor control during voluntary movement
Incomplete muscle tone, shoulder elevation, retraction, shoulder abduction, ext rotation, elbow flexion, forearm supination, wrist and finger flexion
Extensor Synergy
Shoulder protration, horizontal adduction, internal rotation, elbow extension, forearm pronation
Modified Ashworth Scale for Grading Spasticity
0 - no increase in muscle tone, 1 - slight increase in muscle tone (stuck), 1+ - slight increase in muscle tone (mild tone through ROM), 2 - Marked increase in muscle tone through most of ROM; joint easily moved, 3 - muscle tone making passive movement difficult (spastic), 4 - affected parts in rigid flexion or extension
Fugl-Meyer Assessment (Brunnstrom)
Evaluates motor function, sensation, balance, ROM, and joint function on 3pt scale
0 - cannot be performed
1 - partly performed
2 - performed without problem
Reflexive Grading Scale
0 - No evidence of muscle contraction, 1 - hypo-reflexive (decreased but present), 2 - Normal, 3 - hyper-reflexive (increased reaction), 4 - Clonus (repetitive shortening of the muscle after stimulation)
Brunnstrom Stages of Recovery (Movement Approach)
1 - Flaccidity, 2 - Reflexive synergies, 3 - Vol motion in synergy, 4 - Vol motion deviating from synergy, 5 - I from synergy, 6 - Isolated joint movement
PNF: Diagonal Patterns
Performed in all 3 planes D1 F: Combines shoulder flex, add, ER D1 E: Combines shoulder exten, abduction, IR D1 F: Combines shoulder flex, abd, ER D2 E: Combines shoulder exten, add, IR
CIT: Constraint Induced Therapy
Person needs some motion, not appropriate if completely flaccid, trying to promote better control and recognition of affected side of the body
Ishemic CVA
Blockage of cerebral vessel; Most common (appx. 80%)
Hemmoragic CVA
Rupture of cerebral vessel; Caused by HTN, arteriovenous malformation or aneurysm
