CSF uses:
India ink, cryptococcal antigen
Direct examination of tissues
Histology stain, fluorescent antibody (selected organism)
Direct examination of skin, hair, nails, and other selected specimens
KOH/KOH with calcofluor white
Hyphal elements - width, hyaline vs dematiaceous; septate vs sparsely septate; special supplementary media
Culture - 22 to 25 deg C growth = yeast/mold
Culture at 30 deg C
Suspect dimorphic
Pink to salmon pigment, pellicle present in broth, pseudo or true hyphae negative
Rhodotorula, Custobasidium spp.
Pink to salmon pigment, pellicle present in broth, pseudo or true hyphae positive
Sporobolomyces spp.
Cream, buff, or white pigment, encapsulated yeast negative
Hyphae present
Cream, buff, or white pigment, encapsulated yeast positive
Cryptococcus
Hyphae present, assimilation of raffinose positive
Saccharomyces cerevisiae
Negative for raffinose, germ tube positive
C. albicans, dubliniensis, africana
Hyphae positive, arthroconidia
Trichosporon spp., Geotrichum spp. or related genera
Hyphae positive, anneloconidia
Saprochaete spp.
Spindle-shaped blastoconidia
Ustilago spp.
Negative for germ tube, collarette requires lipids for growth negative
Candida spp.
