a retrospective investigations of an event aht ahas already occurered. Thies analysis allows for future action to be prevent fugure errors
what is a failure mode and effects analyze FMEA?
a proactive method in which you prospectively analyze the design of the system to prevent future errors

when you make an error because you forgot to include something
what is an error of commission?
an error when something is mistakenly done incorrectly like giving the medication for the wrong indication, giving someone something they are allergic to. Not renally dosing medications

what are phase I trials?
they give low doses of the compound to healthy volunteers to test teh safety and the pharmacology of the drug.
what are the phase 2 trials?
they give healthy volunteers (usually) different doses of the medication to test out the effectiveness of the medications.
what are phase 3 trials?
they test the medication out in a larger group of people to show whether the effects of the medication show the same SE/effectivness as previously shown.
