Why is it imperative that damaged equipment be repaired and lost equipment replaced as quickly as possible?
It may be difficult or impossible to perform certain tasks with damaged equipment or without lost equipment
Who must be immediately notified by a member that becomes aware of a lost or damaged FD equipment or apparatus?
Their Supervisor
Who will supervisors who become aware of lost or damaged equipment or apparatus notify?
On duty battalion chief
What is the Supervisor responsible to do?
Fill out a lost/ damaged/ destroyed equipment form
Forward the form to their battalion chief and the assistant chief via email before the end of their shift
What will the shift battalion chief or designee assess about the loss or damage of equipment?
Whether or not theft or vandalism has occurred
If the shift battalion chief assesses that theft or vandalism of equipment has occurred who will they notify?
The Police Department
The Assistant Chief
What must be filled out for any theft of city property?
An incident report
If the lost or damaged item is replaced from existing inventory what should be done?
Note it on the report
What will occur for the deliberate or willful misuse, theft, loss, damage, and/or destruction of any tool, equipment, or other device owned by the department or another city department or another agency or private individual?
Appropriate disciplinary action up to and including termination
What may be required of any member responsible for the loss or destruction of any equipment and/or apparatus as a part of the disciplinary process?
May be required to reimburse the COD for the costs to repair the equipment and/or apparatus
