Lidocaine invitations
- Cardiac arrest from VF/VT
- Stable VT, wide complex tachycardia’s of unknown type, wide complex PSVT
- Block increase of ICP due to intubation
- Anesthetic flush prior to I/O
Lidocaine contraindication
Heart block
Lidocaine precautions
- Liver disease
- CHF
- Respiratory depression
- Hypovolemia
- Shock
- Heart block
Lidocaine dosage and administration for cardiac arrest from VF/ VT
- 1-1.5mg/kg IV/IO bolus
- For refractory VF, may give additional .05-.75mg/kg IV/IO, repeat in 5-10 min; max dose = 3mg/kg
- ET dose; 2-3mg/kg in 10ml NS
Lidocaine dosage and administration with perfusing stable VT, wide complex tachycardia, and significant ectopy
- .05-.75mg/kg up to 1-1.5mg/kg IV/IO
2. Repeat .05-.75mg/kg every 5-10 min to max dose of 3mg/kg
Lidocaine dosage and administration for maintenance infusion
Mix 1gm in 250 ml = 4mg/ml or use premixed solution at 2-4mg/min MICRO DROPS / MIN -----MG/MIN 60-----4 45-----3 30-----2 15------1
Lidocaine dosage and ministration for RSI premedication due to ICP
1.5mg/kg IV
Lidocaine dosage and administration for IO anesthesia
Adult: 20-50 mg IO prior to infusion
Contraindicated in pediatrics

1mg/kg IV/IO, or 2-3mg/kg in 5ml NS ET
Lidocaine adverse reactions
- CNS: dizziness, confusion, Paresthesias, seizures, slurred speech
- CV: hypotension, bradycardia
- EENT: tinnitus, blurred vision
