Beta adrenergic blocker with some alpha block effect.
Suppled
100 mg/ 20 ml (5 mg/ml)
Adult Dose
10-20 mg slow IV push over 2 min; doses can be repeated every ten minutes until the desired supine blood pressure is achieved.
Max adult dose
300 mg
Pediatric dose
Safety in children has not been established.
Action
- Beta blockade leads to effects on SA and AV nodes, and ventricular muscle that leads to bradycardia, delay in AV conduction and depression of cardiac contractililty.
- Alpha blockade leads to vasodilation, decreased peripheral resistance, and orthostatic hypotension.
Indication
Hypertensive emergencies (hypertension with evidence of end organ dysfunction: angina, CHF, stroke, renal failure) and pregnancy induced hypertension.
Contraindications
Hypotensive/shock, CHF, 2 and 3 degree heart block, Asthma, Bradycardia, and any known drug interaction.
Side effects
Bradycardia, Hypotension, Syncope, Dyspnea, Heart block, CHF, Dizziness, Bronchospasm.
