Osmotic Agent
Supplied
3% Sodium Chloride in 500 ml bag.
Adult Dose
250 ml bolus over 15 minutes, may have to bolus quicker in some clinical situations.
Pediatric Dose
Contact medical control.
Action
Creates a osmotic gradient to decrease intracranial edema.
Indication
Evidence of cerebral herniation in patients with increase ICP: unconscious patient with unilateral pupillary dilation and or posturing.
Increased ICP as a result of TBI, intracranial bleeding or ischemic stroke.
Contact medical control before administering.
Considerations
Use largest bore IV available. Label bag to indicate hyper-tonic solution. Watch for extravasation.
Side effects
Thrombophlebitis. Electrolyte disturbances. Acid-base disturbances. May induce CHF exacerbation.
