Adult Dose

A

Heparin Bolus: 60 units/kg IVP
Heparin Drip: 12 units/kg/hr
Mix one 5,000 unit Heparin (1 ml) in 49 ml of NS or D5W. Mix will equal 100 units per ml.

A

Hemorrhage. Severe live disease. Severe hypertension. Known allergy to Heparin. Thrombocytopenia. Hemorrhagic stroke in the last 4 weeks.
