Drug Class
Slow Calcium Channel blocker / antagonist
Description
slows conduction, increases refractories in AV node, cause coronary and peripheral vasodilation.
Onset and duration.
Onset = 2-5min Duration = 1-3hr
Indications
A-fib, A-flutter, Mutifocal atrial tachycardia, PSVT (Adensosine is the first choice for conversion of PSVT)
Contraindications
Sick sinus syndrome, 2nd and 3rd degree heart block, Hypotension (systolic <100), A-fib or A-flutter with WPW syndrome or a short P-R interval, Currentt use of Beta blockers, V-tach, Acute MI.
Side effects
1st and 2nd degree AV block, Bradycardia, Hypotension, CHF, Syncope, Ventricular dysrhythmias, N and V.
Adult Bolus Dose
1st dose: 0.25mg/kg IV over 2 min.
2nd dose: Can repeat on 15 min 0.35mg/kg IV over 2 min.
2nd dose is indicated if no response to 1st dose.
Adult Maintenance Infusion
Mix 125 mg in 100 ml of solution. Infuse at 5-15 mg/hr. Titrate drip rate to acceptable heart rate.
Pediatric Dose
NOT FOR PEDS
Additional Information
Do not administer in the same IV line as Furosemide without first flushing the line.
