It is bad incision due to:
- Sloughing & necrosis which occurs at the center part of the incision (area of least blood supply) - exposure of internal carotid artery → infection & hemorrhage → death.

Excision of all lymph node groups removed by radical neck dissection with preservation of one or more of following structures:
- Spinal accessory nerve.
- Internal jugular vein.
- Sternomastoid muscle
