9
Q

How does parietal cells make acid

A

1) H20 and CO2 combine via carbonic a hydrate to from H2C03
2) H2CO3 dissociates to give H+ and HC03-
3) H+ moves into the H-K pump which brings K+ into parietal cell
4) HC03- enters the blood in exchange to Cl- via Cl-HCO3 exchange on the basolateral membrane (alkaline tide)
5) Cl- enters into the stomach lumen to combine with H+ to form HCl

16
Q

Which cell produces secretin

A

S cells

17
Q

Which cell produces cck

A

I cells

18
Q

Where is s cells and i cells located

A

Duodenum

19
Q

What are the pharmacological (drug) inhibitors of acid secretion

A

Proton pump inhibitors e.g omeprazole

H2 receptor antagonist e.g cimetidine and ronitidine

20
Q

What are the 4 phases of gastric acid secretion

A

Basal
Cephallic
Gastric
Intestinal

21
Q

What enzyme does ach stimulate

A

Pepsinogen

22
Q

Which cell secretes pepsinogen

A

Chief cells

23
Q

What happens to pepsinogen when it is secreted

A

Converted to pepsin

24
Q

What is the role of pepsin

A

Initiate protein digestion

25
Q

What inactivats pepsinogen

A

Little acid production

26
Q

What is the protection of gastric mucosal mediated by

A

Prostaglandins

27
Q

How is prostaglandins made

A

Precursor of arachnoidic acid
COX-1 gives prostaglandins for protection
COX-2 fives prostaglandins for inflammation

28
Q

What is the role of NSAID

A

Supress inflammation and mucosal protection by blocking COX-1 and COX-2

29
Q

What does helicobacter pylori do

A

Express the enzyme urease

30
Q

What does the enzyme urease do

A

Convert urea to bicarbonate ions which neutralises the stomach acid

31
Q

What can helicobacter pylori cause

A

Gastritis
Gastric and duodenal ulcers
Gastric cancer

32
Q

What is the treatment for helicobacter pylori

A

2 antibiotic and proton pump inhibitors

33
Q

Which vitamin absorption is intrinsic factors necessary for

A

Vitamin B12

34
Q

What is pernicious anaemia

A

An autoimmune atrophic gastritis directed against parietal cels and stop absorption of B12
