What is meant by ill health and specific needs?
Treatment and care for people with specific needs and ill health take place in a range of settings and are carried out by a range of different people, including those who have had no training, such as family members.
Where is care given?
Care for people with specific need and ill health is given in 6 key settings: Hospital/ Day care units/ Clinics GP surgeries Hospices Residential and nursing homes At home (domiciliary care) The workplace The people who provide care in different settings have different roles. for example a nurse may specialise in early years or in adulthood care, and be part of a team in a hospital ward, a clinic, home care or hospice care. A health and safety officer may give advice on safety at work for a person with specific needs and ill health who needs particular forms of access or safeguarding.
Who provides the care?
Care is provided through four key roles Doctors Nurses Midwives Healthcare assistants These medical staff help people who are ill to get better or learn to manage their conditions. A Midwife might work with a family where a baby has inherited an illness. People in later adulthood might have a range of health issues, with multidisciplinary team meeting to discuss best overall options.
What is the nurses role?
When caring for people with specific needs who are ill, nurses should follow the correct procedures in the context of multidisciplinary teams and in communication with the family.
Follow the correct admission procedures, assessing specific needs.
Provide correct information when answering questions from patient and family members.
Use appropriate language when discussing illness with patient and family members.
Ensure that patient preferences are respected and specific needs catered for.
Provide and administer correct dosage of medication.
Ensure notes are complete and accurate.
Prepare patient for surgery where needed.
Monitor recovery.
Prepare care plans that are appropriate for specific needs, with clear support in place.
Ensure patient discharge forms are correctly completed.
