What is meant by access to services?
Adults and children who need health and social care services may encounter issues that prevent access to services. There is a process in place to define what services are suitable and what can be offered. Three criteria are used to decide which services, if any, individuals receive: referral, assessments and eligibility criteria.
What is meant by referral?
People who need health care usually make an appointment with their GP, where their healthcare needs are assessed. The GP may refer them to a specialist doctor in a hospital or to medical staff at a clinic.
What types of clinics are there?
Clinics provide outpatient care. They are usually located in hospitals and support many types of health need. Examples of clinic support are specialised baby care, ear, nose and throat issues, asthma/allergy issues, genitourinary problems, sleep issues, glaucoma, diabetes, dermatology and cardiac issues. Outpatients do not stay in hospital overnight and are usually treated in day clinics.
What is meant by assessment?
People who may require personal care will have a needs assessment which will be carried out by social services. It will consider:
Their needs and how these impact on their care.
The things that matter to them, for example help with getting dressed.
Their choices and goals, for example, if they wish to take up a new activity.
The types of services, information, advice, facilities and resources which will prevent or delay further needs from developing, helping the person to stat well for longer.
The needs of their family.
The limitation of cost and service provision.
What is meant by eligibility criteria?
Local authorities use needs assessments to assess whether people meet the eligibility criteria for personal care. they consider whether the person’s needs: arise from or are related to a physical or mental impairment or illness. Make them unable to achieve two or more of the specified outcomes. Impact significantly on well-being as a result of being unable to meet these outcomes.
An adult is only eligible for personal care if they meet all three of these conditions.
What could be individual needs and specified outcomes?
Prepare and eat food and drink Maintain personal and home hygiene Go to the toilet appropriately Dress appropriately Move around inside and outside Access personal relationships to avoid loneliness Access occupations such as work, training or volunteering. Use local facilities and transport Care for others such as a child
