Patient with fever, erythema marginatum rash or subcutaneous nodules.
Migratory polyarthritis in wrists, ankles, knees.
Elevated ESR, CRP.
Acute rheumatic fever
Won’t always present with the prodrome having strep.
Prodrome of low grade fever, cough anorexia, lymphadenopathy.
Pink or light red nonconfluent maculopapular rash that starts on the face and spreads to trunk. Rash spreads rapidely and does not coalesce or darken.
Can have small red macules or petechiae on the soft palate.
Rebella (German measles)
Caused by HSV 6
High fever, child appears well during febrile phase.
Classic rash of rose, pink, macular or maculopapular blanchable rash that starts on the trunk and neck before spreading to the face. Only viral exanthem that starts on the trunk.
Roseola (exanthema subitum)
Caused by the aedes mosquito
High fever, headache, retro-orbital ocular pain, biphasic rash with flushed skin,
Tx is supportive care with Acetaminophen
Dengue fever
Tx for Hantavirus?
supportive
Hantavirus comes from inhalation of aerosolized deer mouse feces, saliva or urine.
MC rabid animals?
Bats MC
raccoons, skunks, and foxes.
Tx for ramsey hunt syndrome?
Valacyclovir and prednisone
Herpes zoster vesicles on the side or tip of nose
Hutchinson sign

Comes from dairy products or deli meats.
Causes bacteremia and or meningitis
Avoid deli meats in third tirmester.

Tx IV ampicillin or penicillin
What is the tx for a patient with extreme pain out of propartion to physical exam. Bue hemorrhagic bullae followed by gangrene.
Fournier gangrene of the perineum.
Nec Fasciitis
Immediate and aggressive surgical debridement + antibiotics
How do you Dx Lyme disease?
ELISA followed by Western Blot
Tx for a painful genital ulcers with painful inguinal lymphadenopathy
Gram stain shows a gram negative fastidious coccobacillus.
PCR for Herpes is neg.

Common in countries withh poor access to adequate water and sanitation.
Transmitted by human fecal contaminated soil.
