What is Henoch Schonlein Purpura?
IgA mediated small vessel vasculitis.
What does Henoch Schonlein Purpura preced?
Viral Upper Respiratory Tract Infection
What are the 4 key features of Henoch Schonlein Purpura?
Purpura
Joint pain
Abdominal pain,
Renal involvement
How does the purpura present?
Palpable purpuric rash (with localized oedema) over buttocks and extensor surfaces of arms and legs
How does the joint pain present?
Polyarthritis- knees and ankles
How does the renal involvement present?
Features of nephritis (IgA nephropathy) may occur e.g. haematuria, proteinuria renal failure
Where do you get immunoglobulin deposition with Henoch Schonlein Purpura?
Gut
Joints
kidneys
What is purpuric rash due to?
Inflammation and leaking of blood from small blood vessels under the skin
How is Henoch Schonlein Purpura diagnosed?
Clinically
Urine dipstick
Blood pressure
What would you find with a urine dipstick in someone with Henoch Schonlein Purpura?
Proteinuria
How is blood pressure affected in Henoch Schonlein Purpura?
High blood pressure- Hypertension
What is the management of Henoch Schonlein Purpura?
Supportive management- Analgesia, hydration and rest
What type of condition is Henoch Schonlein Purpura if there is no renal involvement?
Self-limiting condition
What will 1/3 of patients with HSP have?
A second episode of HSP
What will 1/5 of patients with significant proteinuria have?
Long term renal impairment
