What form is required to obtain data for flash lookouts/general broadcasts?

If a member is unable to transmit over the radio a lookout or broadcast, what should the member do?
Use a telephone when possible and call 911.

When should a flash look-out be broadcast?
ASAP, but in all cases, within 5 minutes of arrival at the scene.
When should a general lookout be broadcast?
ASAP, but in all cases, no later than 10 minutes after the flash look-out.

Critical missing person look-outs should be broadcast how often?
15 minute intervals for 1 hour.
If additional broadcasts for a missing person critical are needed, how often should they be broadcasted?
1 hour intervals for four hours, then 2 hour intervals for eighteen hours.
Officials that request repeat broadcasts shall follow up their verbal requests with
a teletype prior to the end of their shift
