WHAT IS FUROSEMIDE
a loop diuretic
inhibits reabsorption from the ascending limb of the loop of henle in the renal tubule
monitoring
electrolytes
adverse effects?
hypokalemia- dangerous in CVD and patients with caridac glycosides (dignoxin), in hepatic impairment this can precipitate encephalopathy
contraindicated in?
renal failure due to nepthron toxic drugs
severe hypokalaemia and hyponatreamia
hepatic impairment- hypokalaemia may precipitate hepatic encephalopathy- use potassium sparing diuretic to prevent e.g. amiloride, spirinolactone
high doses or IV can cause tinnitus and deafness
anuria
examples of potassium sparing diuretics
amiloride
spironolactone
eplerenone
who should not receive these drugs?
CVD patients and those treated with cardiac glycosides
hepatic impairment- encephalopathy
enlarged prostate- urinary retention can occur
hepatorenal syndrome- may reduce diuretic effect and increase side effects
why shouldnt it be used in hepatorenal syndrome
hypoproteinaemia may reduce the diuretic effect and increase risk of side effects
