Depression and Mania Flashcards
Pathophysiology of Depression
decreased brain levels of NE, 5-HT, and DA
Treatment Phases of Depression
acute- 6-12 weeks- remission
continuation- 4-9 months after remission, prevent relapse, eliminate residual symptoms
maintenance- 12-36 months to prevent recurrence
Tx for Depressive Disorders
SSRIs- first line
SNRIs
Bupropion
Examples of SSRIs for Depression
paroxetine
citalopram
MOA of SSRIs
blocks SERT and inhibits reuptake of serotonin
Advantages of SSRIs
higher TI- safer
no cardiovascular SE from M or a block
SE of SSRIs
sexual dysfunction- 5-HT2
GI- 5-HT3
Examples of SNRIs for Depression
Venlafaxine
MOA of SNRIs
blocks SERT and NET to inhibit reuptake of serotonin and NE
Bupropion MOA
blocks reuptake of DA and NE
Bupropion is also used for…..
smoking cessation
SE of SNRIs
sexual dysfunction
SE of Bupropion
higher risk of seizures with high doses
no sexual side effect
Examples of TCAs
amitriptyline
imipramine
MOA of TCAs
blocks SERT and NET to inhibit reuptake of serotonin and NE
SE of TCAs
ANS side effects
low TI- dangerous
block H1- sedation
MAOI Examples
phenelzine
tranylcypromine
MOA of MAOI
irreversibly inhibits MAOa- metabolizes and increases lvls of NE and 5-HT
inhibits MAOb- metabolizes and increases lvls of DA
SE of MAOIs
ANS effects from M and a1 block
many drug interactions
serotonin syndrome with drugs that increase 5-HT
Examples of Mood Stabilizers- Bipolar
Lithium
Valproic Acid
Carbamazepine
MOA of Lithium
inhibits inositol monophosphatase and inositol diphosphatase
inhibits glycogen synthase kinase-3B (GSK-3B) that increases hippocampal levels of B-catenin
Pharmacokinetics of Lithium
low TI
onset of 5-7 days
eliminated by kidney- t1/2 of 24 hrs
competes with Na for renal reabsorption- diuretics increase toxicity
SE of Lithium
hand tremor, edema
nephrogenic diabetes insipidus
hypothyroidism
Lithium and Pregnancy
Ebstein’s anomaly of the new born- disformed tricuspid valve
MOA of Valproic Acid
blocks NA and Ca channels
facilitates GABA
Pharmacokinetics of Valproic Acid
onset of 3-5 days; faster than lithium
SE of Valproic Acid
life threatening pancreatitis
Valproic Acid and Pregnancy
spina bifida- neural tube defects
SE of Carbamazepine
Stevens- Johnson Syndrome
