Significant incidents occurring within the four geographic operations bureaus shall be reported PROMPTLY via ________ following the incident and DURING the shift in which the incident occurred.

The appropriate:
-Operations Bureau Commander
-Assistant Bureau Commander
-Chief Deputy of Emergency Operations
-Designated command staff.
Messages sent via Department e-mail are not encrypted. As such, all e-mails must be free of any Protected Health Information (PHI) that could constitute a ______ breach.

(At no time should the name of any patient be included within an e-mail or anything else that could serve as a personal identifier including residential addresses, photographs, occupations specific to known individuals, etc.)
Significant incident types that require e-mail notification shall include:
-Incidents where dignitaries, celebrities or VIPs are involved (use vague reference to their recognition such as actor, athlete, or politician)
-Significant fires or media events
-Significant EMS incidents
-Saving of a life (including AED use with a successful recovery, return of spontaneous respirations, major patient recovery from a physical rescue, etc.)
-Incidents involving homeland defense issues and multi-agency incidents.
-Public interest events (i.e., unusual rescues of people or animals, etc.).
-Significant LAFD traffic accidents or significant accidents involving a public agency.
-LAFD members injured at incidents or at non-incidents if the injury requires medical treatment.
-Fire fatalities.
-Technical Rescue events.
-Significant hazardous materials incidents.
-LAFD interaction with political or noteworthy groups visiting fire stations or interaction at field events (e.g., congressional, city council, etc.).
-Public service acts performed by LAFD that are outside the direct duties of the Department (e.g., providing a supplemental water supply to a hospital to allow it to stay in service during a water system failure, etc.).
-Closures of public facilities due to fire prevention issues (i.e., overcrowding at a facility that is commonly known to the public, etc.).
-Large scale or significant events due to the type of incident or other mutual aid incidents. VIP contacts, standbys and escorts.
-Allegations of serious or unlawful on-duty misconduct.
-Other incident information deemed noteworthy by the involved Incident Commander.
EMS CAPTAINS shall utilize the significant incident reporting process when forwarding report forms for the documentation of:
-Interfacility Transports (excluding routine STEMI IFTs)
-Multi-Casualty Incidents
-Significant EMS Incidents
-Witnessed Cardiac Arrests
(Existing forms should continue to be completed and secured as an attachment to the e-mail report.)
Members or officers responsible for submitting the significant incident report will normally be the _________.
