How is consent signed without Topaz?

A

Once the consent is generated, leave the signature fields blank and click Accept & Print. Obtain the appropriate signatures on the paper form and then send the form to be scanned into the patient chart. The method for scanning will differ for each clinic.

How do you sign consent with Topaz?

A

Click the Sign Here field for the Patient signature/Date and time. A signature box opens for the patient to sign. Once the signature is obtained, Click next to advance to the next field. Note that there are fields for patient, witness, interpreter, and practitioner signatures. When all signatures have been obtained, click accept.

Where do you go to review the consent?

A

The consent should be linked to the encounter. To view signed consents, check your Chart Review. Signed consents may be reviewed in the consents or media sections, depending on how they were obtained.
