A

If you work in a department where medications are stored in a pyxis, you should select orders with the bed icon. These medications are checked by pharmacists at the hospital and will function more like
inpatient (bed) orders. Once ordered, the administration steps are the same.

Rights of medication:
Right patient – always use two patient identifiers. State first and spell last name, date of birth. Compare to the electronic medical record.
Right drug – compare to what is on the MAR

Right documentation
Complete documentation in timely manner

A

If the route is different than what is listed, you would need to cancel the current order and place a new medication order with the correct route.

A

Patient meds we need to put in the NDC written in the med in the comment section, clinic meds should be on formulary and we would look up the NDC in the med admin page

What is the billing rule called “Incident to” mean?

A

applies to all government payers (Medicare/Medicaid). The billing provider must be present in the clinic/building. If Dr. Docson ordered a monthly injection but when the patient returns in 30 days Dr. Docson is out of
the office, he cannot be listed as the billing provider. This is when the billing provider would need to be changed to another supervising physician. Check with your site leadership for specific details.

Why is it important to have a billing provider for patients that bring in their medications?

A

for patient supplied medications it is important to be sure that this is checked. If not, the patient will be billed for the medication when we save our documentation.

If a medication is given in the office on repeat after a certain amount of days (persistent medication), how do you document this

A

This is similar to a standing lab order

How many days do you have before and after the date of administration for a persisten medication?

A

five-day window before and after to be administered

What is the one exception for persistent medications that can be administered at different timings?

A

hormonal medications.
Hormonal medications have specific rules per drug.

What needs to happen if you need to administer the persistent medication more than 5 days before or after the scheduled due date?

A

you must get a verbal order from the provider to administer it. This conversation must be documented in a progress note, using the SmartPhrase .vomed.

What happens if you do not administer a one time medication?

A

Provider will not be able to sign the one time visit

How can you double check that a one time or persistent medication was administered?

A

Assess & Plan tab. Notice that the CAMs on med management list are greyed out and listed as completed.
