A
Non-necrotizing infection of skin and subcutaneous tissue

Etiology
A
GABHS Staph. aureus H. influenzae, Strep. pneumoniae (children, pre-vaccine) Gram-ve anaerobes (immunocompromised) Vibrio vulnificus

Risk Factor
A
- Trauma - laceration, insect bites
- Skin inflammation- eczema
- Lymphatic occlusion
- Immunocompromised
- Venous insufficiency

Clinical features
A
- Cardinal s/s of inflammation
- irregular margin (not well demarcated)
- erysipelas (well demarcated)
- violaceous colour, bullae (vibrio vulnificus)*aquatic

Investigation
A
- FBC - leucocytosis
- CRP - Inflammatory marker, treatment respond monitoring
- Blood c/s

A
- X-ray
2. US: look for occult abscess

Management
A
Pharmacological
- Antibiotics:
- beta lactam penicillin
- cefazoline, cefuroxime - Antifungal:
- if preexisting - Anti-pyretics
Non-pharmacological
1. Elevation and immobilization: reduce swelling
