Atypical Parkinsons Flashcards
1
Q
What is atypical Parkinsons?
-conditions that come under atypical Parkinsons
A
Parkinsonism - bradykinesia, cogwheel rigidity, postural imbalance
Atypical Parkinsons - progressive disease that presents similarly to PD but do not respond to levodopa
- DLB
- progressive supranuclear palsy
- multiple system atrophy
- corticosabal syndrome
2
Q
Risk factors for atypical PD
A
Spontaneous
Long term drug exposure
Trauma
3
Q
Differences in presentation between PD and APD
A
APD
- initially bilateral signs
- faster progression of symptoms
- loss of dopamine receptors
- normal gait
- wheelchair sign - darting out of wheelchair without thinking about safety
PD
- initially unilateral signs
- loss of dopamine
- football gait pattern
4
Q
Presentation of progressive supranuclear palsy
A
Postural instability, falls -stiff, broad based gait Impaired vertical gaze => issues going down stairs Bradykinesia Frontal lobe dysfunction
Vertical forehead wrinkling - Procerus sign
Blepharospasms, eyelid apraxia
Poor response to Ldopa
5
Q
Presentation of multiple system atrophy
A
Parkinsonism Autonomic dysfunction -ED -postural hypotension -atonic bladder
Orofacial dystonia
Cerebellar signs - DANISH
- dysdiadochokinesia, dysmetria
- ataxia
- nystagmus
- intention tremor
- slurred staccato speech, scanning dysarthria
- hypotonia
