Echinocandins (Caspofungin, Micafungin, Anidulafungin (IV)
A
- Cell-wall inhibitor, inhibits B-1,3-D-glucan synthesis
- Use for Invasive Candida (esophagitis), candidemia and Invasive Aspergillus
- Don’t use for C. neoformans and endemic fungi

Ampho B, Nystatin
A
- Binds and forms pore at ergosterol
- Broad spectrum: Use for Cryptococcal meningitis, mucormycosis, resistant Candida, serious endemic
- Nephrotoxicity, ototoxicity, electrolyte changes, infusion-related toxicity

Fluconazole
A
- inhibits ergosterol synthesis at 14α-demethylase step
- Use for yeasts and endemic fungi. Cryptococcus at maintenance phase.
- Don’t use for Candida krusei
- hepatitis, long QT, CYP 450 inhibition

A
Ampho B + azoles (vori-, posca-, itra-), Fluconazole for CNS cocci

Voriconazole, Posaconazole, Isavuconazole
A
- inhibits ergosterol synthesis at 14α-demethylase step
- Use for molds (asp. fumigatus), endemic
- Visual changes (vosi), hepatitis, long QT, CYP 450 inhibition

Ampho B + Flucytosine (induction)
Fluconazole (maintenance)

- Chain terminator; 5-FU disrupts pyrimidine synthesis
- Use for Cryptococcus neoformans (together with Ampho B)
- Do not use alone
- BM suppression (cytopenia)
