3 main techniques for mandibular field blocks
- IAN nerve block
- gow-gates
- vazirani-akinosi (closed mouth)
what must always be done before administering
- make sure mucosa is taut
- aspirate (so that you don’t penetrate blood vessel)
steps to administering the IAN block
- palpate external oblique ridge until you get to coronoid notch
- move medially until the internal oblique ridge
- move back laterally to make tissue taut
medial to lateral
- pterygomandibular raphe
- pterygotemporal depression
- internal oblique ridge
- external oblique ridge / coronoid notch
where is the needle inserted
pterygotemporal depression (triangular shaped)
where does the needle contact during gow-gates
neck of condyle just below insertion of temporalis m
when do you use gow-gates
when the IAN block doesnt work
when do you use vazirani-akinosi
when there is limited opening of the mouth or if you cant see the intraoral landmarks
