Antidysrhythmic (class V) / Nucleoside
MOA for Adenosine
Adenosine (A1) R-agonist - Slow AV conduction by activation of G protein 2nd messenger, decreasing CaMP, and inhibiting calcium entry into the cardiac pacemaker cells
Pharmacokinetics for Adenosine
Onset : Immediate
Peak : immediate
Duration : 30-45 seconds
Half : 6 seconds
Indications for adenosine
SVT (PSVT) / Narrow Complex Tachycardia
Diagnostic maneuver : stable narrow complex tachycardia
Wide regular monomorphic tachycardia
Contraindications for Adenosine
2/3 heart blocks, sick sinus syndrome, polymorphic VT ( Torsades )
Poison / chemical induced SVT ( ex. Cocaine)
Side effects for Adenosine
Facial flushing, HA, chest pain, dizziness
Adult Dose for Adenosine
1st dose : 6mg rapid IV/IO (proximal vein) followed by 20cc NS flush
2nd dose : 12 mg rapid IV/IO (proximal vein) followed by 20cc NS flush
Pediatric dose for adenosine
1st dose : 0.1mg/kg rapid IV/IO (proximal vein) max 6mg
2nd dose : 0.2mg/kg rapid IV/IO max 12mg
