Individual name tags
2”x 1/2” hard plastic tag w/ velcro backing, shall have first initial and last name, as well as 2 letter dept. identifier.
Primary Passport
flexible velcro 2”x 4” with permanently affixed unit ID at the top and a removable unit ID tag 2”x 1/2” at bottom
Unit Roster
TheunitrosterisahardbackedpieceofVelcro(2”x 4”). A permanently affixed unit designator will be at the top, a removable unit identifier (2” x 1⁄2”) will be at the bottom.
Apparatus passport drop-off board
The driver’s door of each Engine, Truck and Squad will have a hard plastic board (8.5” x 11”). The front of the board will have Velcro attached so to receive at least six soft passports.
Helmet Identifier
If applicable to agency, each riding member will have an approximately (3”x2”) unit identifier attached to the frontshield of their helmet. This helmet identifier must be in place anytime personnel are assigned or are riding a specific piece of apparatus.
Unit Command Board
hard plastic board designed to hold passports and assist company officers in organizing and documenting an incident if the company officer assumes command. The unit command board will have Velcro attached to it to provide for the placement of soft passports and removable unit identifiers.
When will a unit command board be used?
when company or command officers are assigned to a division/group or branch.
Chief Officer’s Command Board
The chief officer’s command board is a large plastic board that has Velcro or magnetic attachments and large writing areas to track units and assignments for an entire incident.
Where will the primary passport be carried?
On the unit officer’s person. Each agency can determine the location on the officer person to be carried
Where will unit passport be located?
Affixed to the officer’s door or passenger door of rescues
Where will passport be placed on staff/command vehicles?
Driver’s door
Unit officer name tag is placed

Driver’s name tag
placed under the officer’s tag.
When is a tag placed upside down?
if the driver is staying outside of the IDLH.
When will implementation of passport accountability system occur?
Any incident that requires use of SCBA, or FF at risk of becoming lost, trapped, injured or at discretion of IC
Default initial accountability location on multi-company incidents
First arriving engine or truck
Remote entry point passport drop off
When units enter structure from location remote from Side A, units drop passports at first arriving unit at remote locations door. Announcement must be made of remote accountability location.
Who is responsible for passports and tracking of those units at remote entry points
Group/division officer or IC as soon as possible
Crews should remain intact when possible. Minimum crew size will be ?
Two or more
All crews entering the hazard zone must be ?
Directly supervised by an officer, acting officer or designate by dept. policy
Under certain conditions, crews may split up, providing that
- does not reduce level of safety for crew members.
- no one operating in hazard zone alone
- each crew has portable radio and appropriate PPE
- accountability officer updated for accurate tracking
- IC authorizes the change
If a radio fails while in hazard zone
Crew should exit immediately unless another radio is with crew. attempt to secure replacement should be made
Who shall account for all personnel under their command and be prepared to report this when called by IC?
Group/Division officers
Unit officers shall report what during PAR check?
Total number of people assigned to their unit, and indicate the number of people operating outside the hazard zone.
Who is responsible for units/personnel not assigned to group/divisions, i.e. outside truck person, engine driver
Incident Commander
After all units have PAR, the IC shall
Document the time and note on the command board
If any personnel cannot be accounted for, the group/division officer shall report the status of missing personnel as ?
“Unknown” and give IC their last known location and NAME.
In case of missing person, Group/Division officers shall
Initiate search procedures within their own area.
All groups operating on the incident shall maintain their current positions and assignments unless directed by IC.
IC may initiate PAR check to check the status of personnel anytime deemed necessary. Some situations in which shall be done include:
- member/crew missing or trapped
- when unit/crew cannot be contacted in Hazard Zone after 3 consecutive attempts
- Sudden hazardous change on incident scene
- Conditions deteriorate to point evacuation ordered
- change of mode of operation
- Incident declared under control
Accountability Group
As the incident escalates to level that multiple Accountability Officers are assigned
When command staff expands, who will Accountability Officer be assigned to?

Lost/Missing Firefighter
An absent member of any crew will automatically be assumed lost/trapped in the hazard zone until determined safe
