Quality of care
Ability of health care services to increase the likelihood of desired health outcomes and consistency with current professional knowledge
Harmful incident
an incident that resulted in injury to the patient
-a fall
Near miss
an incident that did not reach the patient (no harm resulted)
-Patient started falling but you put a wheelchair behind them
No-harm incident
an incident that reached the patient but caused no discernible harm
-Put the wrong cream on the patient, got the wrong meal
adverse event
(Patient safety incident )– could results or did results in unnecessary hard to a patient
Swiss cheese model
Each slice is a barrier put in place to try to make the health care safer.
Active failures – the wholes come into contact with the patient and negatively impacts them
Failure mode and effect analysis (FMEA)
– prospective, looks to the future. Try to forsee potential failures
Root cause analysis (RCA)
– take data and try to make improvements based on trends
TeamSTEPPS
Based on team memebres knowing strengths and weeknesses of everyone on the team
Critical Thinking
Integration of knowledge and experience.
