A

Only Platoon Commander, Pct of Occurrence, will prepare Parts A and B

If not available, then

Patrol Supv, will be Investigating Supervisor and prepare Part B only

DO, Pct of Occurrence, will designate a 2nd supervisor to complete Part A. If none available, then the DO will prepare Part A himself.

Part C only prepared by cop if he’s requesting LOD designation. If unable to do so, CO/DC only one that can prepare.

Aided prepared by injured cop. If unable to, then have another cop prepare.

They will send the Trauma Counseling Team
To scene.
