IV Cannulation Flashcards
What is cannulation
Insert line into vein => deliver fluids, medication, blood
Equipment needed
Torniquet Sharps bin Gloves Alcohol gel and wipes IV cannula - correct size relative to size of vessel and fluid Adhesive dressing 5ml saline
Introductions
Introduce yourself, confirm patient ID
Any problems in past with needles
Explain procedure, consent
Uncover arm
Contraindications
Past mastectomy
AV fistula
Identification of suitable vein
Wash, gel hands
Non sterile gloves
Torniquet
Find springy vessel in non-dom hand/arm
Insertion of cannula
Clean site with alcohol wipe => let it dry
-DO NOT TOUCH SITE
Use your non-dom hand to keep skin taut, anchor vein to prevent it from rolling
Place cannula needle in line with direction of vein few mm below entry site
Bevel pointing up (15-30o)
Insert needle at angle, watch for flashback
Advance needle parallel to vein
Remove stylet 5mm to avoid piercing post vein wall => further flashback
Advance cannula into vein
Release torniquet, place finger over vein above tip of cannula to prevent bleeding as you remove the stylet completely
Disposal of needles
Lock needle => into sharps bin
Cap the cannula
Flush and secure cannula
Cover insertion site, immobilise with sterile non occlusive dressing Label with current date Flush with 5ml saline -look for swelling, leakage -ask for discomfort or pain
Documentation
Complete cannulation insertion chart
Document patient details, date, time, reason, gauge, site, date of removal
Your name, signature, grade, contact details
Follow up care
Assess for signs of phlebitis every day
Must be removed immediately if infected, or no longer needed
Replace every 3 days
Signs of infusion phlebitis
Erythema Swelling Pain along path of cannula Induration, palpable venous cord Fever
