w6 safe blood collection Flashcards
cause of bacteremia
bacteria multiply in blood stream faster than they can be eliminate
- abscesses
- dental extraction
- surgery
- brucellosis
- typhoid
when are blood cultures ordered
FOU fever of unknown origin
PUO pyrexia of unknown origin
what do blood cultures determine
- presence and extent of infection
- identity of organism
- antibiotic to use
when are blood cultures ordered
- before anticipated fever spike
- after spike
- before antibiotics
- from more than one site
collection set
blood cultures collected as sets one aerobic vile one anaerobic vile -all bacteria detected ordred 2 -3 times at intervals increases change of isolating organisms and ruling out contaminations
consecutive collection technique
collect anaerobic and aerobic from one arm then immediately collect both from the other from same site and you indicate on bottles which arm R or L – acute symptoms need antibiotics immediately
time interval technique
s=collect at a second site at certain time 20-30 mins after – limited amount of blood drawn is a lot
collection notes (8)
- inspect bottles; contamination cloudy ness cracks bulging etc
- performed at bedside-processed within 2 hours
- room temp time of collection- incubated 27
- mark vial before collection so you know volume.-skin scrupulously cleaned and disinfected.
- repalpating not recommended
- note insufficient specimen on req-split between two vials inoculate aerobic bacteria only or some say inoculate only one vial so aerobic
- nsq not sufficient quantity note on req
- when preparing follow same patient id and vein assessment
extra steps to take blood
-clean with alcohol then clean with povidone iodine pad or chlorhexidine gluconate swab for 30 seconds using friction scrub in 8-10 cm area
-use butterfly needle
-mark fill line on bottle -then remove plastic cap from vials and cleanse stoppers with fresh alcohol pad and allow to air dry
engage vial in holder and fill-*keep tube in downward position to avoid back flow
-order of draw
-invert
** document which arm blood came out of
order of draw
aerobic anaerobic then other blood tubes if required
blood syringe collection
sterile 10 or 20ml syringe used when veins fragile. transfer quickly to to vial there is no anticoagulant in syringe
