Urinary Tract Infection (UTI) Flashcards
Causes?
E. coli, proteus, klebsiella, s. faecalis, pseudomonas
Which cause are common in in-patients/catheterised?
pseudomonas
Lower UTI features?
dysuria, frequency, haematuria, suprapubic discomfort, urgency
Upper UTI features?
systemic features, loin/back pain, vomiting, rigors, preceding cystitis, shock/sepsis
Investigations?
urinalysis – haematuria, proteinuria, leucocytes, nitrites. MCS – leucocytes, WBC casts +
Bloods: U&E, FBC,
Blood Cultures
USS for upper UTI
Drug Mx of lower UTI in pregnant women with symptomatic bacteriuria?
7 days course of nitrofurantoin (trimethoprim only after 12 weeks)
Drug Mx of lower UTI in asymptomatic pregnant women ?
7 day course of trimethoprim/nitrofurantoin + 2 urine culture before Abx and 1 after
Drug Mx of lower UTI in non-pregnant women?
Trimethoprim or nitrofurantoin for 3 days
Drug Mx of lower UTI in men?
2 week course of trimethoprim or nitrofurantoin
Drug Mx of LUTI if catheterised?
ciprofloxacin or co-amoxiclav 7 days
Mx of Upper UTI?
Hospital admission
IV antibiotics: gentamicin bolus
Oral antibiotics: cefuroxime (ceph.) or quinolone (e.g. ciprofloxacin)
