- a clinic reading persistently above >= 140/90 mmHg, or:
* a 24 hour blood pressure average reading >= 135/85 mmHg
Investigations?
Bloods: FBC, U&E’s, lipids, HbA1c
ECG
Urine dipstick
Fundoscopy
Drug Mx of HTN < 55 yrs?
Step 1: A (ACE-i)
Step 2: A + C (calcium channel blocker)
Step 3: A + C + D (Thiazide Diuretic)
Step 4: If K <= 4.5 mmol/L add spironolactone
If K > 4.5 mmol/L increase dose of D
Drug Mx of HTN >= 55 yrs or Afro-Caribbean?
Step 1: C
Step 2: ARB + C (calcium channel blocker)
Step 3: A + C + D (Thiazide Diuretic)
Step 4: If K <= 4.5 mmol/L add spironolactone
If K > 4.5 mmol/L increase dose of D
SE of ACE-i?
Cough
Angioedema
Hyperkalaemia
SE of Ca channel blockers?
Flushing
Ankle swelling
Headache
SE of thiazide diuretic?
Hyponatraemia
Hypokalaemia
Dehydration
