Causes? (4)
A
Staph aureus (70%)
N.gonorrhea (sexually active young people)
Strep.
Osteomyelitis

Risk factors? (4)
A
Rheumatoid arthritis
Immunocompromised
Steroid use
Prosthetic joints

Q
Presentation?
A
One joint affected (usually the knee)
red, hot, painful, swollen.
Movement not tolerated.
Fever, rigors.

Investigations?
A
FBC, U&E’s, CRP, ESR
Blood cultures
JOINT ASPIRATION (yellow, purulent, cell count >10 000, neutrophils, gram positive.
X-ray (later - shows bone destruction)

Mx?
A
Analgesia - NSAIDS
Antibiotics:
Flucloxacillin and benzylpenicillin
IV 2 weeks, oral 4 weeks
